This point-and-click adventure blends horror, puzzles, and mystery in Boston six years after the apocalypse began. The world is in permanent darkness, and the dead do not stay dead. Players control Mia as she explores the city, abandoned buildings, parks, a cemetery, sewers, a library, and a medical research facility.

When Mia encounters other survivors, she can help them, loot them, kill them, or insult them. Her choices shape her fate, but death is permanent: the game has no saving or loading, and only one life is available. Other activities include finding records to play on a jukebox and taking part in three mini-games.
